Eastside Utility makes it simple to apply for new water service or request service at a different address. Complete the form online or use one of the options below to submit your application.
Online Application
- Complete the water service contract directly on this page.
- Upload a copy of the signer’s driver’s license within the form (upload two if co-signer).
- Submit the completed form online.
- After submission, click the link below to pay the $50.00 service fee.
- Click here to pay the new service request fee.

Fax: Fax the completed form and your drivers license to 423-490-9516, then call 423-892-2890 to pay the $50.00 service fee.
